When a client raises the topic of an assistance animal, we are being asked to make one of three distinct clinical determinations. Our standard clinical protocol for an assistance animal assessment has four elements. A structured clinical interview that establishes DSM-5-TR or ICD-11 criteria for the presenting condition. This is the foundation; skipping or skimping here is where clinicians get into trouble. Document specifically how the condition substantially limits one or more major life activities — sleeping, concentrating, caring for oneself, working, interacting with others. This is where most clinicians are insufficiently specific. The letter should articulate why this animal (not animals generally) meaningfully reduces which specific symptom. California AB 468 requires a minimum 30-day established clinical relationship before a letter can be signed. Several other states are moving toward similar standards. Notice what this contains: a specific statute citation, a specific major life activity, a specific animal's name, a specific measurable change, and a specific legal basis for the request. A clinician who issues letters to anyone with $99 is not practicing psychology; they're issuing transactions.
